§ 2-914. Issuance of duplicate checks when originals lost.
The chief financial officer shall have authority to issue a duplicate city check upon receipt of proper evidence in writing from the payee that such check has been destroyed, mutilated or lost and was not presented to the bank for payment. Upon notice from the payee, it shall be the duty of the chief financial officer to issue a stop payment order to the bank on which the check was drawn and, upon receipt of written notice from the bank acknowledging the stop payment order, the chief financial officer shall issue a duplicate check and notify the bank to honor such check when presented for payment. It shall further be the duty of the chief financial officer to maintain a record of all duplicate checks issued.
